Dr Okun,

Lately my mom keeps leaning to her right when seated and she's stiff as a rock when I try to move her back to the center. Now when she walks without assistance she will sway like she's drunk and lean to one side which results in her losing her balance and fallen a few times. Do you know what this happens? Does this have anything to do with her PD or dementia?

Impossible to know without seeing her in person, but you should be sure her medication doses are enough and intervals close enough. Also, physical therapy may help. I would optimize and see how she does.
