DBS lead misplacement

Dr. Okun,


My mother-in-law (72 yrs-old) had one-side DPS placement 4 yrs ago in China with no good response and currently is told that the lead is placed a bit deep and may need to be pulled back for location adjustment. We are also told that the adjustment of lead location may have complications including higher risk of brain hemorrhage. Could you please tell how higher risk of complications/hemorrhage for DPS lead adjustment? Is there any other alternative management?

This is a tricky question. The risk is pretty low if they just pull the lead back into place. Pulling the lead back often does not work, and in that case if a second surgery is necessary than the same risks for DBS will be in play (bleed, infection, stroke, etc.).
