Steerable DBS

Doctor Okun,

     What are your thoughts about steerable DBS?  Do you feel it has the potential of reducing the negative risks of speech articulation, impulse behaviors and/or mood that are associated with STN?

I think we need more data and that certainly it has the potential to do great things.  Side effects such as pulling, swallowing or voice would be at the top of my list.  Also, gait and balance.  Not sure if it will address cognition or ICD as well or if at all.  Again, need data.  Most important is that we implant the lead in the best spot as a well placed lead will be the first step...

Dear Doctor , 

How do you determine whether the placement of the leads are good or bad. Are there any objective criteria or does it depend on the opinions of the experts in the  team?

There are no objective measures at this time however whether the lead is steerable or not is less important than if it is in general well placed.  We use imaging (MRI-CT) plus thresholds at each contact to determine the location.  Ask your doc about this approach.
