Great question. Here is the best way to describe them.

Extended Release products are made up of Controlled Release and Sustained Release. A Controlled Release product releases a substance at a constant rate over a longer period of time. A Sustained Release product releases a product over a longer period of time but not at a constant rate.

The best way for me to describe it would be the following:

Let's use the example of a Sustained  Release  tablet. Once this is is taken is broken down the medication will usually be absorbed slowly at first, then at a higher level, then decreasing amount over time. To visualize it, I use the comparison of  a bolus injection. A large amount of medication is injected into the muscle, then the medication is slowly released into the body until none is left.

A Controlled Release Tablet, on the other hand, will release a certain amount at a set time. For example it may release 0.5mg per hour every hour until it is gone.

Many times all of these terms are interchanged and i hope the explanation above is claer.
