How long for meds to work

Hi all my dad was diagnosed with pd  on wed, totally devasted but not surprised, he is 63 and is starting 12.5 mg sinemet 3 times daily, how long would it take for meds to improve symptoms ? Thks a mil x

 This Tom 64yrs. is your dad's dose 25-100 mg broke in 1/2 at 6am, 12 noon and 6pm say.  So he gets 50mg per dose.  The doctor is a giving your dad a starter dose. (min. safe dose) To see if your dad gets side effects to the sinemet.  Sinemet is the safest and  the drug used most often after 60. This is a very low dose. If it is too low he will probably not notice much.  Another important issue with many of us is a protein meal to close to the taking the dose. Protein can effect the uptake of your dad's dose. IMO your dad's doctor is starting with my same dose and I feel your on track to start.

The best thing you can do is keep notes dose time and meal times and if there is any change positive or negative(Negative upset stomach, head ache, rash and so on.  Positive symptoms better. (tremor, stiffness, speed walking and so on). By keeping notes you can be clear when you call his doctor. Ask your doctor all the ? you have.

I can tell you in my case I noticed some progress on symptoms pretty early with the starting dose and no side effects.  My doctor gave it a week and then raised my dose to  1, 25-100 sinemet every 6hours. and that was like day and night to for me.  But I had symptoms for 6 years before I got to a movement specialist. I lived with PD and a chiropractor finally told me to see a movement doctor. 59 diagnosed and I take at 64 900 mg a day.

We are all different and we progress at different rates.   Your dad and his doctor make the call on how to treat PD.  It is nice to to have someone like you to help and be there for him.   Sorry I can't give more advise. Some things are just trial and adjust until the correct treatment is attained.   Tom

Hi Tom thank you for your reply the dose says 12 .5mg 3 times daily , is this starter dose and how long would it take to see improvement? Dad has right hand  tremor since last may, slow movent since 1and half yr it took me a long time to convince him to get 2nd dr opinion he is very old school as 1st dr said essential tremor, he believes everything !! I knew he wasnt right,  but now at least we have a diagnosis , its just so scary

Sam,

Sinemet is the brand-name for carbidopa-levodopa. The lowest dose that it comes in is a yellow tablet that has 25mg of carbidopa and 100mg of levodopa. Levodopa is the drug that actually helps with Parkinson's. The carbidopa is added to help the levodopa cross what is known as the "blood-brain barrier." It also reduces the nausea that is a common side effect of levodopa (l-dopa for short). With all of that being said, it sounds as though your father is on a 1/2-tablet every 6 hours while awake schedule - which we would call a "50mg dose" (referring to the l-dopa).  It is doubtful that he will see any improvement on that dose, especially considering that levodopa doesn't last more than a few hours in the body. As Tom was saying, the doctor is most likely trying a very small dose just to see how he does before increasing his dose to a more therapeutic level.

As for the short answer to what you are asking, if your father takes a dose that is effective for him, he should see improvement within 45 minutes of the first dose. Right now, the real question is, "what dose is effective for him?" Every patient is different, so there is no definitive answer to that question other than to say that it would most likely be at least 1-tablet every 4-6 hours.

Sam Hello again I kinda thought your dad was old school.  Kinda a know someone like that.  My wife adds hardheaded. LOL   Things take some time and when the weather gets nice talk your dad out of the house to go for hikes.   Exercise is a must. Stretching to save a full range of movement.   Find out if there is a rock steady Boxing program in your area.  The program is geared for us PWP.  Also tell him a old school hard head said "if you don't use it you lose it". Referring to arms and legs. LOL   Probably just got myself in trouble?

Thks for your reply guys :) Tom he is def old school everything is grand kinda guy lol . Exercise is big part of his life ,he goes the gym  5 times a week mon to friday,  will check out the boxing thing it sounds fun, he might like that as he used to be black belt karate !  Thks for informing me bout ldopa, the neuro said it should take 2 weeks to see improvement, so i wasnt sure if the dopamine needs to accumulate first if you know what i mean , thks guys much appreciated Leanna
